Medical guidelines have been developed by Fortuna Mining Corp. to ensure visitors remain in good health, while meeting local regulations, when traveling to the Lindero Mine, due to the high elevation.

The following requirements must be met for any visitors staying up to 7 days at Lindero.

1. Clinical physical examination (prioritizing cardiovascular system and lungs)
2. Blood test (Hemoglobin, Hematocrit, Glucose and Glycosylated hemoglobin (only for Diabetics)
3. Electrocardiogram (for people of 45 years old and over)

Tests must be completed by a medical professional in your country of residence.

Results of the above tests, along with other required info must be filled out and approved by your doctor or medical professional using the medical form available below.

This form is mandatory and required in order to complete your visitor registration.
